چکیده

THE RISING NUMBEROF PATIENTSWITH ADVANCED HEART FAILUREWHO RECEIVE DURABLE MECHANICAL CIRCULATORY SUPPORT (MCS) DEVICES requires improved recognition of device complications (1). Imaging is invariably needed because the clinical presentation is nonspecific. In our institution, we perform both a baseline echocardiographic study and a pump speed optimization test when left ventricular assist device (LVAD) dysfunction is suspected (2). Additionally, we obtain a contrast electrocardiogram (ECG)-gated multidetector cardiac computed tomography (MDCT) study to screen for complications in patients without significant impaired kidney function. With fast acquisition, superior spatial resolution, and superior image quality, ECG-gated MDCT could directly visualize the entire MCS systems (except for the pump interior) and

Neurological dysfunction associated with mechanical circulatory support: complications that still need attention.

While infection and bleeding are clearly the most prevalent complications in mechanical circulatory support, neurological complications can perhaps be the most devastating. These complications are a leading cause of morbidity, mortality, increased hospital stay, and cost. These complications impact not only the patient, but also the entire health-care delivery team, and the patient’s friends an...

Impact of abdominal complications on outcome after mechanical circulatory support.

BACKGROUND Mechanical circulatory support (MCS) is life sustaining for patients with end-stage heart failure. Most devices require abdominal wall transgression, creating a potential for abdominal complications. The incidence and impact of these relatively underreported complications are unknown. METHODS A retrospective review was performed on 179 patients who received MCS therapy from 1999 to...

Systemic inflammation in patients with heart failure.

AIMS We hypothesized that chronic heart failure as a model of systemic hypoxia may result in systemic inflammation. The signs of a systemic inflammatory response should disappear after successful mechanical circulatory support using biventricular assist device systems. METHODS AND RESULTS Plasma levels of cytokines (IL-6, IL-8, TNF-alpha) and soluble adhesion molecules (sVCAM, sE-, sL-, sP-Se...

Mechanical circulatory support: reality and dreams experience of a single center.

Because of the increasing number of patients waiting for heart transplantation and the decreasing number of donor organs, mechanical circulatory support has become a generally accepted therapeutic option. Several high-tech devices developed in the last 15 years differ in terms of location, kind of support, and driving units. They are suitable for different patients and their therapeutics object...

Double lung procurement from a donor supported by a left ventricular assist device.

Over the past several years, the selection criteria for marginal donor lungs have been extended. However, brain-dead patients with implanted mechanical circulatory support systems have not yet been considered as potential organ donors for lung transplantation. Our report presents the first successful procurement and transplantation of donor lungs from a patient supported by a left ventricular a...
